!!IMPORTANT!! Please reference this calendar for tryout times/dates. Checking the calendar is the best way to stay up to date with any changes.
Please fill out this form to sign your child up to tryout for our travel ball, Wildcat, program. This does not guarantee your child a spot on the team. This simply signs them up for the tryout. This is NOT to register your child for the recreational program.
Please double check that your e-mail is input correctly, this is how we communicate with you! If it is input incorrectly you will not get our emails and will not know dates/times/changes of the tryout schedule, or if your child makes the team!
* By submitting this form, I, the parent/guardian of the registrant, a minor, give my permission for him/her to participate in the Charlton Wildcat program. I recognize the possibility of physical injury associated with basketball, and hereby release, discharge and/or otherwise indemnify Charlton Wildcats, Charlton Youth Basketball, its sponsors, volunteers, and associated personnel, as well as the owners of the basketball facilities use for the program, against any claim by or on behalf of the registrant as a result of the registrant's participation in the basketball program. I declare the registrant is in good health and able to participate in physical activity.
